I hereby give permission to the staff of the above GenZ programme to administer medically prescribed medication to my child. I understand that the staff will record each administration of medication. I acknowledge that all care will be taken and will not hold GenZ responsible.
I hereby notify GenZ that my child carries medication with them and will self-medicate when necessary. I understand that my child is to let staff know when they self-medicate so that a record may be kept and any further instructions followed.
Off Site Excursions GenZ programmes will at times have special trips and activities that occur away from the main site. Please read the Excursions section of the Parent Information for further details.
I hereby give GenZ permission to transport my child off a GenZ designated site of operation if and when required i.e. evacuation, group trip etc.
I authorise GenZ to publish Facebook, Blogs or online sites:
Photographs containing his/her image
Video clips containing his/her image
Audio clips containing his/her voice
Copies of his/her work
I agree that this consent shall continue until I withdraw my consent by notice to the supervisor.
I acknowledge that the information contained herein is confidential and, pursuant to the Privacy Act, will only be strictly used by the GenZ team to effectively care for my child and not used or distributed for any other purposes.

I hereby give my permission for the GenZ staff to treat my child if a minor accident occurs. In the case of a more urgent matter I understand an ambulance will be called first then I will be notified.
Fees are our only source of income. To operate efficiently we require that fees be kept up to date.
TERMS AND CONDITIONS
Invoices/statements will be issued weekly. If fees remain unpaid for more than two weeks a plan to pay overdue fees will be negotiated if necessary.
Parents will be notified of the OSCAR fee and subsidy available from Work and Income at enrolment.
While you are waiting for a WINZ application to be approved, you are still required to pay your fees in full. WINZ can take up to 4 weeks to process and we ask that you contact them regularly regarding the status of your application. If WINZ do not accept your application, you are still liable for all fees incurred by your child’s booking.
We reserve the right to refer any unpaid accounts and dishonoured cheques to our Debt Collection Agency. Once the matter is referred to them, you will be liable for any and all collection fees including agency fees and commission we would have to pay for this service
Without prejudice to any other remedies GenZ may have, if at any time you are in breach of any obligation (including those relating to payment) GenZ may suspend or terminate the enrolment and is absolved of its other obligations under the terms and conditions. GenZ will not be liable to you for any loss or damage that you may suffer because GenZ has exercised its rights under this clause.
In the event that your automatic payment is dishonoured for any reason then you shall be liable for any dishonour fees incurred by GenZ.
Where you are an individual the authorities under this clause are authorities or consents for the purposes of the Privacy Act 1993.
You shall have the right to request from GenZ a copy of the information about you retained by GenZ and the right to request GenZ to correct any incorrect information about you held by GenZ
I declare that I have read this document fully and that the information given above is true. I acknowledge that in order to keep my place at GenZ that I need to keep my account up to date.
I have attached the $35 deposit for the Holday Programme.
I am aware that any default by me for the payment of outstanding fees may result in debt collection action and all costs associated with this action will be at my care.
I acknowledge that it is my responsibility to advise GenZ immediately of any change in the above information.
