TUITION AND FEES: We agree that all tuition and fees are to be paid in full or kept current and that the school may immediately discontinue admittance into class or suspend attendance if the account is not current, depending upon the delinquency. In the event of voluntary removal by the school and/or parents, any unresolved balances may be turned over to the school"s legal representatives and litigated through the court system until payment is received. Until all financial matters are resolved, no records will be forwarded and/or no information will be shared with another school. RECORDS/DOCUMENTATION: We accept responsibility for obtaining in a timely manner and submitting to the school thorough, accurate documentation of all special needs (i.e., custody papers, medical conditions/allergies, attention or learning problems, educational/psychological testing and/or physical limitations) concerning our child"s safety/education at school. We understand that these must be submitted, discussed with, and evaluated by the administration before our child enters school. We will not hold Cathedral Christian Academy responsible for failing to respond appropriately if not given full and/or current documentation, copies of all relevant legal change documents., and all other forms of notification. We understand that failure to provide or cooperate with such correspondence may result in immediate dismissal. FUNDRAISER POLICY: We will participate in three fundraisers each school term which helps C.C.A supplement the cost of tuition. It is mandatory that each parent/child partake in each fundraiser by selling/raising in the amount of $200 per fundraiser. It is understood that if the selling requirements are not met, we will be responsible for paying a fundraiser fee of $550.00 per academic school year. We also agree to donate monthly supplies such as paper towels, trash bags, cleaning materials, toilet tissue, plastic forks, spoons, knives, paper plates, hand sanitizer, etc, as an attempt to supplement the monthly tuition cost. FIELD TRIPS: We hereby give permission for our child to accompany school groups on supervised trips and community walks. We understand that reasonable safety precautions will be taken and will not hold the school, employees, and volunteers responsible for accidents or injuries our child may sustain. We and our child/ren will cooperate with the school"s safety programs and procedures including dispensing of medicines, playground rules, and other safety guidelines. Please understand that we will give your child proper supervision and take the proper precautions. ELECTRONIC DEVICES: We agree with the confiscation of any and all electronic devices such as cell phones, mp3 players, video games, radios, cd players, etc, if brought to school as a violation of school policy. All devices will be confiscated and not returned until the last day of school. Demanding our child’s property before deemed so, will result in our child’s immediate dismissal from C.C.A. MEDICAL: We give C.C.A permission to administer emergency first aid for injury or illness and to transport our child to the hospital required by law according to the school"s location as perceived necessary. We know that reasonable efforts will be made to contact us concerning such emergencies. We will provide to C.C.A at least three other emergency contacts who have agreed and have authority to act on our behalf. This includes authorization to pick- up/remove our child from school. We give consent for the school or outside agencies to administer any other health/learning screening/evaluations available to students. ADVERTISEMENTS: We hereby agree to allow C.C.A the use of our child"s photo or other recorded likeness, voice or written work for educational and/or publicity purposes, and release the church, school and all employees/volunteers from any liability regarding the use of such. DISCLOSURE: Cathedral Christian Academy admits students of any race, color, national and ethnic origin to all rights, privileges, programs, and activities generally accorded or made available to students at the school. It does not discriminate on the basis of race, color, sexual or gender orientation or identity, national or ethnic origin in administration of its educational or admissions policies, scholarship, athletic and other school administered programs.
