I, the undersigned principal, agree to participate in the Code.org programs selected below.
I also understand Code.org’s goal to expand computer science instruction within my school district. As such, I will assist Code.org with introductions to the appropriate district-level staff and serve as an internal advocate for the Code.org program.
After you submit this form we will reach out to you on whether you've been accepted due to limits on capacity.
Program Details:
As part of my school’s participation in the pilot program, I will:

Identify at least one teacher to teach the program(s) selected below during the 2015-2016 school year. Our preference is two teachers per school.

Agree that the teacher will participate in all professional development phases relevant to the course(s) selected below (see pages 3-4 here (http://code.org/files/DistrictPartnershipPlan.pdf) for professional development model)

If selected below, ensure that students are able to register for Exploring Computer Science or Computer Science Principles, at the same time as other classes for the 2015-2016 school year and that the course is placed on the master schedule

One administrator & counselor from the school will attend in-region professional development if available.

Publicize the course to all students and parents

Provide the materials necessary for the teacher to teach the course
As a member of this pilot program the school will receive from Code.org:

Professional development for teachers:

Teachers will receive stipends for the participation and successful completion of each of phase of the professional development

As part of the summer phase of professional development, teachers will participate in an in-person workshop either in the local area or at a location to-be-chosen by Code.org.

All curricular resources including:

Promotional materials targeted at teachers, parents and students (if desired)
Minimum technology requirements per school:

One lab consisting of X computers (Mac or PC) per student (where X is the maximum Code.org course class size)

Online connectivity at a minimum of 10MB/sec

Ensure that all emails sent from *@Code.org are whitelisted by district filters.
Code.org will not be responsible for costs associated with meeting the above described technology requirements.
Code.org will not receive any individual student achievement or demographic data from the school or district. However, through Code.org’s tutorial platform, Code.org may collect student data from students enrolled in courses within and outside of the school or district. This may include data such as student logins as well as student progress and achievement on online tutorials within Code.org’s tutorial platform. Students participating through Code.org’s tutorial platform can use accounts set up by the school, district or by their teachers, and will be allowed (but not required) to provide email addresses. Code.org has a Privacy Policy which governs Code.org’s use, sharing and collection of data that Code.org has access to or collects through its programs, including its online tutorial platform.
