I give permission for my child to attend NMIC school classes. It is understood that every precaution will be taken for the safety and well-being of my child, but in the event of accident or sickness, Newmarket Islamic Centre, its staff and its volunteers are hereby released from any liability.

NIC SCHOOL POLICY:
1. Parents are responsible for their child/children actions and behaviour.

2. Parents need to be aware that the Islamic school teachers are here to facilitate Islamic Learning & give direction to the parents and it’s their responsibility to provide the children with Islamic teaching and backup the learning of Sunday school.
3. All Parents are responsible for their children if any accident happens.
4. Upon registration, I understand and accept the terms and NIC reserves the full right to cancel this registration and they may accept any or none of the applications at their sole discretion.
5. By signing up my child(ren) to the Newmarket Islamic Centre, I give permission for my child(ren) to participate in all the activities organized by them and for my child(ren) to receive emergency medical treatments, if necessary. I hereby grant permission for the NIC to release the PROVIDED personal and medical information about my child(ren) to a third party in the event of a medical emergency. I also excuse the NIC, its owners, management and operators, staff and tutors from all liabilities, and all claims, if any, arising from any accident, injury and/or loss in any form, be it financial or physical, that may have been caused by or arisen from such participation of my child(ren) in the Newmarket Islamic Centre.
